Aftercare

Posthospitalization program of rehabilitation designed to reinforce the effects of therapy and to help the patient adjust to his or her environment and prevent relapse.


Outpatient treatment of formerly institutionalized persons within their communities.


The care of a person who has had an operation. Aftercare treatment involves changing dressings and helping people to look after themselves again.


The care of a mother who has just given birth.


Any of a variety of programs designed to provide follow-up care to individuals who have completed a drug-rehabilitation program. Services can include job training, counseling, and opportunities for drug-free social interaction.


Care of a convalescent, chronically ill, or handicapped person after formal acute medical care has ended; may include the provision of special aids or facilities (e.g., handrails, ramp) necessary for living at home.


Long-term surveillance as an adjunct or supplement to formal medical treatment of those who are chronically sick or handicapped. After care includes the provision of special aids and the adaptation of homes to improve daily living.


Care of a convalescent after conclusion of treatment in a hospital or mental institution.


The provision of care and medical attention for individuals recovering from illnesses.
