Code data

A unique symbol (usually alphanumeric) having a one-to-one correspondence with a term or a rubric. Codes may be used on the patient’s bill, for example, to indicate the service for which the charge is shown. Diagnoses and procedures are commonly coded for ease of manipulation by computer.

When data or information has been replaced by a code, it is said to have been coded, i.e., the coding process has been carried out.
