Discharge planning

The work of making a plan for when a patient leaves hospital to live at home.


The process of making sure that arrangements are made outside the hospital to receive the patient upon discharge and to provide the necessary continuity of care.


Any strategy used to facilitate outpatient care for someone currently receiving inpatient health service. It may include the identification of the proposed date of hospital dis charge, a program for the use of professional or familial home health services, a review of the appropriate use of prescriptions and their side effects, instructions for the use of durable medical equipment, and a list of appointments for follow-up care.
