Documentation method using a columnar format to chart data, action, and response.
A charting system based on “focus statements.” A focus may be a nursing diagnosis, a patient concern or behavior, a significant change in the patient, or a significant event in therapy. After the nursing assessment is conducted, a column is created with a list of focus statements instead of nursing diagnoses/problems, and a care plan is written. Nursing notes use focus statements, with the organization DAR: D = data about the patient, A – action (actual or planned nursing interventions), and R = response (patient’s). The system was developed at Eitel Hospital in Minneapolis in 1980. “Focus” is a registered trademark.
