Formulary

A listing of drugs, usually by their generic names, A formulary is intended to include a sufficient range of medicines to enable physicians or dentists to prescribe medically appropriate treatment for all reasonably common illnesses. A hospital formulary normally lists all the drugs routinely stocked by the hospital pharmacy. Substitution of a chemically equivalent drug in filling a prescription by brand name for a drug in the formulary is often permitted. A formulary may also be used to list drugs for which a third party will or will not pay, or drugs which are considered appropriate for treating specified illnesses.

A document containing recipes for the preparation (compounding) of medicinal drugs. An example of such a volume is the National Formulary (NF), an official publication of the United States Pharmacopeia Convention (USPC). Pharmacopeias, issued by various nations, contain not only the recipes, but also standards as to the strengths and purity of drugs (see pharmacopeia). In the United States, USPC also issues the Pharmacopeia of the United States of America (USP), which is a legal standard.

Published jointly by the British Medical Association and the Royal Pharmaceutical Society of Great Britain, with input from the Department of Health, it is distributed to all NHS doctors by the government. A companion volume deals with children. Many hospitals and general practices produce their own formularies for use by doctors working in those facilities.
