A community member who serves as a connector between health care providers and health care consumers. Most of the work is done in the community setting among groups which have traditionally been underserved. The CHW identifies community health problems, develops innovative solutions, and translates the solutions into practice. Sometimes called a “lay health provider”.<\/p>\n
Usually managed by NHS trusts, these are a complex variety of services provided to people outside hospital settings. The key parts are the services delivered by district nurses, health visitors and therapists \u2014 for example, physiotherapists and speech therapists.<\/p>\n
