Disclosure by a health program of all ownership interests in the program. By law, each skilled nursing facility participating in Medicare and Medicaid must supply ownership information to the State survey agency and each intermediate care facility must supply such information to the State licensing agency. Full and complete information must be supplied on the identity of: each person having (directly or indirectly) an ownership interest of ten percent or more in such facility; in the case of a facility organized as a corporation, each officer and director of the corporation; and in case the facility is organized as a partnership, each partner. Any changes which affect the accuracy of this information must be promptly reported.<\/p>\n
