Yellow card system

This is a reporting system used in the UK to document adverse reactions to drugs. When doctors or pharmacists suspect that a prescribed medication (including vaccines), self-administered treatment, or herbal remedy has caused an unexpected or undesired symptom, they complete a yellow card. This information is then gathered and analyzed at a central level. The yellow card system plays a crucial role in identifying issues that may not have been discovered during the initial testing of newly introduced drugs.
