A method of preventing errors in which information relayed to one person is repeated and verified in a slightly different form as a means of confirming its accuracy. For example, a respiratory therapist is asked to administer an aerosol to a patient named “Dabs.” Before giving the medication, he repeats the name of the patient out loud as “Dabs… delta, alpha, bravo, sam. Have I spelled the name correctly?”
